Case Manager
Full-time, Non-exempt
The Case Manager is responsible for providing community-based intervention outreach services and evidence-based education to men who are of childbearing age to decrease the impact of substance use. The Case Manager will work directly with fathers, who are Texas residents in region 8, with the youngest child up to six year old; or expected fathers who have one or more risk factors for a substance use disorder as identified in the Parenting Awareness and Drug Risk Education (PADRES) Screening. The Case Manager will work with Child Protective Services, treatment centers and other community-based organizations in Bexar and 27 surrounding counties to deliver intervention outreach services.
